You must use the CMS form for DME reimbursement, which can be found with instructions at https://www.cms.gov/Medicare/CMS-Forms/CMS-Forms/Downloads/CMS1490S-ENGLISH-Instructions-DME.pdf. BraceLab’s receipt (order confirmation) must be attached which has the correct code for filing (L3908).
Contact your insurer to obtain the correct form to request reimbursement. Following these guidelines does not guarantee reimbursement; that is the decision of each insurer.
Medicare Pricing, Data Analysis and Coding (PDAC) has assigned L3908 as the code to use when requesting insurance/Medicare reimbursement.
If you are an ASHT member, non-product specific coding information may be found on their website: ASHT L-code Information. BraceLab is not a provider. We do not file insurance nor can we provide information on insurance coverage or reimbursement rates. Patients should contact their insurance provider for filing information.
To determine the size of the Push med Wrist Brace needed, measure around the wrist as shown. Select RIGHT or LEFT in the corresponding size from the chart. NOTE: This measurement is only a guideline for determining size. If measurement is near the size break point, only trying on both sizes can allow you to determine the best fit.
Push Braces include a “dot system” to help you secure each strap to the proper place, in the proper order. Simply start with the strap that has one dot on the hook-and-loop pad at the end, and secure it to the hook-and-loop pad on the brace that has a matching dot. Repeat with the strap that has two dots, and so on, till the brace is secured. It’s that easy!
Each brace includes precise instructions for how to wrap each strap, although a professional fitting is always recommended for optimal comfort and support.
The Push® med Wrist Brace can be machine washed on gentle cycle at low temperature. Be sure straps are closed during washing. Washing in a net bag is recommended. DO NOT add bleach. DO NOT dry in dryer or wash in a dishwasher.
