You must use the CMS form for DME reimbursement, which can be found with instructions at https://www.cms.gov/Medicare/CMS-Forms/CMS-Forms/Downloads/CMS1490S-ENGLISH-Instructions-DME.pdf. BraceLab’s receipt (order confirmation) must be attached which has the correct code for filing (L1930).

I am writing to thank you for the wonderful customer service, fast shipping, and most of all, the AMAZING PUSH AFO my husband and I purchased from you. Actually, we've bought 2: The first one was paid for by my insurance. The second we bought because we want to be sure he always has one to use. It's a life changer.
Last Fall my 64 year old husband woke up with foot drop on his right side. We did all the usual, MRIs, a neurologist/EMG, physical therapy, even saw a very respected foot and ankle reconstruction specialist. The eventual diagnosis was a peroneal nerve injury, unexplained, and a prognosis of "probable, eventual, partial recovery" which could take a year or more. In the meantime, he couldn't work his part time job (He's been essentially disabled with severe OA since 2013), couldn't drive, and was becoming very depressed. An orthotist gave him, first, a solid-carbon AFO which led to hideous skin breakdown/pressure sores and left him completely NWB. Once we healed those wounds, they casted him for a "custom" AFO. It took 4 weeks to come in and was horrible, totally unusable. Rigid, hard plastic that went nearly to his knee, hurt terribly, and left him just as disabled as before: He still couldn't drive, the footplate wouldn't t fit in ANY shoe...their suggestion was a pair of $450 custom shoes, which would not solve the issue of pain in any event. He was very discouraged. While his job is not a major factor for us economically, it has been huge for his self esteem and overall outlook. I'm not sure people realize how much harm the loss of independence and mobility can do. It was difficult to watch him retreat into himself.
So I did some research, found the PUSH AFO, and started lobbying his doctors and the orthotist. The docs were pretty interested and willing to prescribe, but the orthotist was dead set against it and actually kind of hostile. We insisted, and I am so glad we did!
My husband has been driving, walking, and back at work since mid-January. None of this would have been possible without the PUSH. I hope clinicians will open their minds to something that looks different but works the way this does. I can't thank you enough for what you've given us. Obviously we hope that after some months my husband won't need an AFO, but until that time, he is functional and living his life. : ) I can recommend the PUSH for comfort, flexibility, ease of use, and cost. It's a truly wonderful thing.

To determine the size of the Push ortho Ankle Foot Orthosis (AFO) needed, measure around the ankle as shown below. Select RIGHT or LEFT in the corresponding size from the chart.

Push Braces include a “dot system” to help you secure each strap to the proper place, in the proper order. Simply start with the strap that has one dot on the hook-and-loop pad at the end, and secure it to the hook-and-loop pad on the brace that has a matching dot. Repeat with the strap that has two dots, and so on, till the brace is secured. It’s that easy!
Each brace includes precise instructions for how to wrap each strap, although a professional fitting is always recommended for optimal comfort and support.
The Push ortho Ankle Foot Orthosis (AFO) can be machine washed on gentle cycle at low temperature. Be sure straps are closed during washing. Washing in a net bag is recommended. DO NOT add bleach. DO NOT dry in dryer or wash in a dishwasher.
